The Clark County Firefighters Local 1908 Health Insurance Trust has partnered with CareHere to provide you and your family with a care center to treat both acute and chronic conditions such as the common cold and the flu, high blood pressure, and diabetes. We also provide annual physicals, health coaching, lab work and much more.

This document has information about the center, including the hours of operation, physicians on duty, instructions to log onto the website, info about the mobile app and more.

To register a new account:

A separate account is required for the member, spouse and any dependents.

To register your account, you will need an access code and a unique user ID.

Go to, and click the "Member Login" link. On the My Health Guide page, click the "I need to register..." link. Use one of the following access codes:

Access Codes:

CCF2 - Members, Spouses, & Dependents
CCMR6 - Medicare Retirees
CCFR4 - Retirees

Proceed to the Identification page and enter your unique ID, birth date, username, password and email to finish creating your account. Future logins will utilize the username and password you create for each account.

What's my unique ID:

Your unique ID is based off the ID number on your Local 1908 Security Fund medical insurance card. There is a 9-digit number on the card that looks like "999-47-XXXX". Your unique ID will use the last 7 digits of this number, and no dashes. The formula to create your unique ID is as follows: Start with four zeros (0000), add it to the last 7 digits of your card ID with no dashes (947XXXX), then add the following digit to the end of the sequence:

1 - member
2 - spouse
3 - eldest child
4 - next eldest
5 - next eldest, etc.

Your unique ID should look something like 0000947XXXX1

  • Fridays: 1:00 pm - 7:00 pm / Ashley Kelley, MD
  • Saturdays: 9:00 am - 1:00 pm / Rotating Provider

Hours are subject to change. If this occurs, changes that affect the established schedule of hours of operation will be communicated to patients.
